Verification of the license or certificate of a chemical dependency counselor or prevention specialist may be obtained as follows:

Via Telephone: Call our office Monday - Friday during normal business hours at 614-387-1110. Be sure to have the individuals name, social security number or license/certification/file number. If you will be seeking verification of more than three professionals, we ask that you make your request in writing and either mail, fax or email the total list to our office.

Via Fax: Document the name, social security number or license/certification/file number of the individual(s) you wish to verify and fax that information to our office at 614-387-1109. These credentials will be verified, and you will receive a returned response via fax within 7 business days.

Via Mail: Document the name, social security number or license/certification/file number of the individual(s) you wish to verify and mail that information to our office at 37 W. Broad Street, Suite 785, Columbus, Ohio 43215. These credentials will be verified, and you will receive a returned response via mail within 14 business days.

Via Email: Document the name, social security number or license/certification/file number of the individual(s) you wish to verify and email that information to our office at credentialing@ocdp.state.oh.us. These credentials will be verified, and you will receive a returned response via email within 7 business days.
