LOCATE A TESTING SITE:

All examinations offered by the Board are computer based and available at a number of regional sites throughout Ohio and our border states. Most exam sites are open Monday - Saturday. For a complete list of computer based testing sites please visit: http://www.isoqualitytesting.com/mlocations.aspx

COMPLETING THE EXAMINATION PROCESS:

Step One Submit a formal application for licensure/certification and have this application approved. Formal applications are available on the "Forms & Applications" page of this website. The Board will notify the applicant once their application is approved.
Step Two

Once application approval is granted, submit an Examination Order Form with the $150.00 examination fee. Examination Fees must be paid in the form of a money order made payable to IC&RC. Checks and credit card payments will not be accepted as methods of payment for an exam fee.

Upon receipt of the Examination Order Form, the Board will pre-register the applicant for the examination and the applicant will receive a username and password via email from the examination provider.

Step Three

Log in to the examination registration system using your assigned username and password and schedule a date, time and location of your choosing for the examination.

Usernames and passwords are only valid for a six month period, so it is important that applicants schedule and take their examination in a timely fashion. Applicants are encouraged to choose their exam date wisely. Fees will be assessed if the applicant reschedules or cancels their examination. Please read the Cancellation/Rescheduling Policy below for further details.

Step Four

Take the examination at your scheduled date, time and location.
Applicants are given 3 hours to complete the examination. Preliminary results of the examination are available to applicants upon completion of the exam and official results are sent to applicants 1-2 weeks following the exam.

Applicants who fail the examination may immediately submit another Examination Order Form and fee to reschedule the examination but will be required to wait 60 days before they are permitted to re-test.

CANCELLATION/RESCHEDULING POLICY:

If an applicant needs to cancel or reschedule an examination they must contact the Board and pay the appropriate cancellation or rescheduling fees. Applicants who cancel or reschedule an exam at least five days before the exam will be assessed a $25 cancellation/rescheduling fee. Applicants who cancel or reschedule an exam less than five days prior to the exam will forfeit their examination fee.

EXAMINATION WAIVER INSTRUCTIONS:

Individuals who hold a license with a scope of practice to provide AOD services in the state of Ohio may request a waiver of the examination requirements for licensure by completing an EXAMINATION WAIVER FORM and submitting it with a formal application for licensure. A copy of the license must also be provided with the application.
