Application and Forms/ Requirements

The Board does not accept paper applications

Online applications are reviewed in the order of “date submitted”.  The review process can take up to 40 business days from the date of receipt due to the vast number of applications we receive.

Continuing Education:  Applicants applying for a Treatment credential must complete courses that are approved by the Board’s CEU approval process or another authorized entity.   College or University courses may also be used to meet the education requirements but do not need to be pre-approved by the Board or another authorized entity.

ONLINE APPLICATIONS

To apply for a certificate, license or endorsement with the Board:

  1. Navigate to the e-License Portal. The e-License Portal is most compatible with Google Chrome or the latest version of other internet browsers.
  2. Register/Login to your Dashboard and select the "Apply for a New License" link.
  3. You will be required to upload and attach all required forms. See forms below.
  4. You will be required to pay the $50 application fee ($35 if you are converting from an LCDC II or an OCPS to a higher license/certificate).
  5. If you are required to send a transcript please send to the TRANSCRIPTS ONLY E-Mail:  transcripts@ocdp.ohio.gov or by mail. 
  6. It can take up to 40 business days to process applications. 

CHEMICAL DEPENDENCY COUNSELOR REQUIREMENTS/FORMS

CDCA REQUIREMENTS/FORMS

  • Must be at least 18 years of age and hold a High School Diploma or GED
  • Forty (40) hours of chemical dependency specific education grid in the required content areas - CDCA Preliminary 
  • Must hold the Preliminary for a minimum of 10 months 
  • Thirty (30) hours of chemical dependency specific education grid in the required content areas - CDCA Renewable
  • Completion of online application

LCDC II, LCDC III, LICDC REQUIREMENTS/FORMS

The following degrees are the initial requirement to apply for the LCDC II, LCDC III and LICDC:
LCDC II - A minimum of an Associate's degree in a behavioral science or nursing OR a Bachelor's degree in any field
LCDC III – A minimum of a Bachelor's degree in a behavioral science or nursing
LICDC - A minimum of a Master's degree or higher in a behavioral science with documentation of coursework in ten specific content areas
 

Remaining LCDC II, LCDC III and LICDC Requirements:

  • One year (2,000 hours) of chemical dependency counseling related work experience (compensated work or supervised internship)
  • A job description identifying clinical functions signed by your Clinical Supervisor
  • 180 hours of chemical dependency specific education grid in the nine specific content areas. 
  • 220 practical experience hours in the twelve core functions. This may be completed as part of your work experience requirements
  • Successful completion of the ADC examination or completion of Examination Waiver Form
  • Completion of online application

CLINICAL SUPERVISOR (CS) ENDORSEMENT REQUIREMENTS/FORMS

  • Hold an active LICDC
  • One year (2,000 hours) of chemical dependency counseling related work experience (compensated work or supervised internship)
  • One additional year (2,000 hours) of work experience as a clinical supervisor of chemical dependency counseling services (Cannot be the same dates as the chemical dependency counseling related work)
  • 30 hours of clinical supervisory education
  • Successful completion of the ADC & CS examinations or completion of Examination Waiver Form
  • Completion of online application

GAMBLING DISORDER ENDORSEMENT REQUIREMENTS/FORMS

Applicants must hold an active LCDC II, LCDC III, LICDC or LICDC-CS license with the Board to be eligible to receive a Gambling Disorder Endorsement.
Applicants must complete 30 hours of gambling specific education in the following content areas:
  • Knowledge of Gambling Disorder
  • Gambling Counseling Practice
  • Special Issues in Gambling Treatment
  • Professionalism in Gambling Treatment

PREVENTION PROFESSIONAL APPLICATIONS

RA REQUIREMENTS/FORMS

OCPS & OCPC REQUIREMENTS/FORMS

The following degrees are the initial requirement to apply for the OCPS, and OCPC
OPCS- A minimum of an Associate's degree
OCPC- A minimum of a bachelor's degree
 
Remaining OCPS and OCPC Requirements:
  • One year (2,000 hours) prevention related work experience (paid or volunteer) – OCPS ONLY
  • One year (2,000 hours) prevention services,  plus an additional two years ( 4,000 hours) in administrating or supervising prevention  services – OCPC ONLY
  • 90 hours of administrative or supervisory education – OCPC ONLY
  • 120 hours of prevention education in the specified domains
  • Completion of 120 practical experience hours in the six performance domain areas
  • Successful completion of the ICRC Prevention Examination
  • Completion of online application
