Conversion Application Requirements/Forms

The Board no longer accepts paper applications. All applications must be completed online.  Make sure when you apply in elicense you type your name as it should appear on your wall certificate. Do not use all lower-case letters or your wall certificate will print your name in all lower-case letters.

Online applications are reviewed in the order of “date submitted”.  The review process can take up to 40 business days from the date of receipt due to the vast number of applications we receive.

Continuing Education:  Applicants applying for a Treatment credential must complete courses that are approved by the Board’s CEU approval process or another authorized CE entity. The education provider must reference the name and approval number for the formal approval body on the CE certificates.  College or University courses may also be used to meet the education requirements but do not need to be pre-approved by the Board or another authorized entity. 

 

ONLINE APPLICATIONS

 

To apply for a certificate, license or endorsement with the Board:

  1. Navigate to the e-License Portal. The e-License Portal is most compatible with Google Chrome or the latest version of other internet browsers.
  2. Register/Login to your Dashboard and select the "Apply for a New License" link.
  3. You will be required to upload and attach all required forms. (PLEASE TAKE THE TIME TO READ/VIEW ALL OF THE MATERIAL THOROUGHLY.)
  4. You will be required to pay the $50 application fee ($35 if you are converting from an LCDC II or an OCPS to a higher license/certificate).
  5. Official Transcripts are required and may be sent to the Transcripts Only E-mail TRANSCRIPTS ONLY E-Mail:  transcripts@ocdp.ohio.gov or by mail. 
  6. It can take up to 40 business days to process applications. 

Conversion Application Requirements/Forms 

IMPORTANT NOTE: All Conversion Applications must be submitted to the Board no less than 30 days prior to the expiration date if you do not renew your credential.  This does not guarantee approval before your current credential expires.   Our review processing can take up to 40 business days, so if you do not renew you could be at risk of experiencing a lapse in licensure while waiting for your conversion approval.  Licensees who received the COVID-19 Extension must renew prior to submitting a Conversion Application.

Conversion Requirements to move up from the LCDC II to the LCDC III License

  • An active LCDC II License
  • Submit an official Bachelors Degree transcript
  • Complete and Submit the Online LCDC III Conversion Application 
  • Pass the ADC Exam (if not previously passed to obtain the LCDC II)
Conversion requirements to move up from the LCDC II or LCDC III to the LICDC License
  • An active LCDC II or LCDC III License
  • Submit an official Master's Degree transcript in a Behavioral Science that contains a minimum of 40 semester hours earned with documentation of coursework in ten specific content areas on the Masters Education Grid  
  • **IMPORTANT** Before you apply, make sure your Master's Degree meets requirements per OAC 4758-5-05(A),(1) 
  • Complete and Submit the Online LICDC Conversion Application 
  • Complete and attach the Master's Degree Education Grid to the online application (all areas must be covered using masters coursework only)
  • Pass the ADC Exam (if not previously passed to obtain the LCDC II or LCDC III)
If using college education: One semester hour = 15 clock hours or One quarter hour = 10 clock hours

Conversion Requirements to move up from the OCPS to the OCPC License

  • An active OCPS License
  • Submit an official Bachelors Degree transcript or higher to the Board
  • 90 hours of administrative or supervisory education 
  • 4,000 hours in administering or supervising prevention services
  • Submit a job description 
  • Complete and submit the Online OCPC Conversion Application 
  • Pass the IC&RC Prevention Exam (if not previously passed to obtain the OCPS)

 

If using college education: One semester hour = 15 clock hours or One quarter hour = 10 clock hours
 
OHIO WORK EXPERIENCE - The required Prevention Work Experience hours for Ohio Applicants must be submitted by the Supervisors through Elicense (See Reporting of Supervision ONLINE under the Certification and Licensure link)
