Individuals wishing to report a possible ethical violation to the Board may do so by completing an Ethics Grievance form and submitting it to the Board office via mail or fax.

Ethics Grievances are reviewed and processed by the Board's Ethics Committee. When processing ethics grievances, the Ethics Committee adheres to the practices outlined in the Disciplinary Action Rule.
Certified or Licensed individual's who have an ethical grievance filed against them will be notified by the Board and granted an opportunity to respond to this grievance. If the Board determines that there has been a violation and decides to seek action against the individual, the individual is given an opportunity to request a hearing in this matter. This process is outlined in the Disciplinary Action Rule.

Any ethical grievance or response form must have a signed ROI ( Authorization for Release of Information) if client name or any client information is to be released to the Board.
